We are in need of a full time, direct hire Clinical Care Manager in St. Paul, MN.  The Clinical Case Manager provides phone-based clinical case management services to individuals and families enrolled in programming. He/she is responsible for managing a caseload of clients struggling with behavioral health related illnesses including addiction, eating disorders, mood disorders, anxiety disorders, trauma, chronic illness, and pain management. He or she assists clients with the development of social and community support systems that promote stability, self-management, and enable the client to function at the highest level possible. The clinical case manager is part of the clinical team and MUST HAVE either a Masters level education in behavioral health field OR bachelor degree with independent licensure to apply. This position functions with a high degree of independent accountability, in accordance with professional and ethical standards and organizational policies.

The hours are M-F 8: 30-5PM.
