PsychPros is searching for a community based rehabilitation specialist in the Cleveland area interested in helping  adults with disabilities learn to  live their fullest life. Working with a supportive and collaborative team, candidates will be helping clients with restoration of social and daily functions, Care coordination, and service linkage.

Essential Duties and Responsibilities
This position will provide home and community based rehabilitative services to adults that consists of mental health interventions, care coordination activities, and collaboration with other internal and external providers to promote positive consumer outcomes. Service delivery is face to face, phone, and telehealth with 90% of contacts being face to face.
Service activities to include:
  • Care coordination of healthcare, behavioral healthcare, and non-healthcare services
  • Referral and linkage to other healthcare, behavioral healthcare, and non-healthcare services
  • Identification of strategies and treatment options related to the individual’s mental health diagnosis (es)
  • Restoration of an individual’s social and daily functioning
  • Crisis intervention as needed
Responsibilities:
  • Engages in monthly all staff meetings and bimonthly departmental meetings to support the effective treatment of individuals within an integrated care model; meets with assigned supervisor for individual supervision twice a month
  • Adheres to a positive team culture and promotes personal empowerment in the treatment process
  • Attends all client meetings and maintains ongoing contacts with other collaborating providers to ensure optimal treatment outcomes
  • Provides quality care through the provision of identified evidence-based approaches and interventions
  • Participates in activities to promote the professional growth and development of staff (trainings, conferences, case consultation)
  • Meets productivity requirements of 5.5 hours daily
  • Maintains client documentation per agency policy and in the electronic health record according to internal standards and external licensing and regulatory standards
  • Provides services and emergency coverage to clients as needed
  • Ability to work a flexible schedule as appropriate to the needs of the clients
  • Actively participates in the agency’s continuous quality improvement process
  • Documents daily work time in the timekeeping system
