Job Description
We are assisting a client in search of Case Managers in the Woodland Hills, CA area.
Qualifications
Case Manager Bilingual English/Spanish preferred, but may not be required
Under general supervision of the Program Supervisor this position provides case management and other related mental health services for persons with mental health disorders.
Depending on the program, may include children and/or adults.
This position is responsible for maintaining a caseload as well as conducting outreach and group work. Completes case management assessments, collaborates with families/significant others, facilitates needed services and linkages such as housing, benefits, employment, education, medical/health care, and/or substance abuse supports.
This position works in the office as well as with clients in the community (field work included). Some work may be conducted by phone or telehealth (but not all).

Minimum Position Requirements:
- BA in social work, psychology, or related field and 2 years mental health experience, or other related degree and applicable experience.
- Ability to understand and interpret information, follow instructions and manage workflow.
- Experience with culturally diverse population
- Strong Interpersonal skills; ability to work effectively with people in a constructive manner within a community-based treatment setting.
- Able to communicate in a clear, precise and professional manner.
- Ability to apply professional judgement in solving difficult issues.
- Able to multi-task in a fast paced environment.
- Current driver' s license, with valid registration, and insurance required (clean driving record) .
- Must be able to clear a Department of Justice and FBI background check as required by the CCL (Community Care Licensing ) guidelines.
- Must be willing to provide services in the office and in the community. Also have ability to effectively communicate via telehealth platform and via phone when needed.
Desired:
- Experience in the provision of case management services for persons living with mental illness.
- Substance Abuse counseling/support experience
- Knowledge of Los Angeles County Department of Mental Health- a plus
