We are currently hiring a Housing Case Manager to work full-time with clients experiencing homelessness in Altadena, CA. This is a TEMPORARY ASSIGNMENT with the potential for converting to a permanent position for a strong candidate.
Qualifications-
Experience providing case management with underserved populations required.
A bachelor’s degree in Social Work, Psychology, or Sociology and a minimum of 1 year of experience in a social services setting is preferred.
Possession of a valid California driver's license and daily access to properly registered vehicle are required.
Pay-
$23.50/hour
Schedule-
Monday-Friday 8:30-5:00

Responsibilities-
Facilitate the quick placement of individuals and families into stable permanent housing to minimize the duration of homelessness.
Provide case management and support for 20-25 households.
Rapidly enroll clients into the program, including gathering documentation needed for enrollment and creating Housing and Services Plans.
Conduct home visits once a month at minimum. Weekly visits are required for any unhoused participants.
Collaborate with Housing Navigators to assist individuals in navigating the lease up process.
Complete and submit monthly rental packets that accurately reflect the household needs as well as progression toward financial independence
Provide supportive services and resource linkage to promote housing retention and wellness.
