PsychPros is a nationally recognized staffing and recruiting firm dedicated to the behavioral healthcare field.  We are currently hiring a Housing Case Manager to work full-time with clients with chronic health conditions who are also living in a permanent supportive housing location in Altadena, CA 91001. This is a TEMPORARY ASSIGNMENT with the potential for converting to a permanent position for a strong candidate. The ideal candidate will be available to start immediately.

Requirements-
  • A minimum of 1 year of case management experience in a social services setting is required.
  • Bachelor’s degree in Social Work or related field is preferred.
  • Experience working with homeless or very low income individuals is preferred.
  • Possess a valid California driver's license and have daily access to properly registered vehicle.  

Benefits-
  • The pay is $23.50/hour
  • The schedule is Monday-Friday 8:30am-5pm

Responsibilities-
  • Provide intensive case management to clients living in a permanent supportive housing program who also have a chronic health condition
  • Conduct intake and enrollment with eligible clients
  • Assist clients with accessing temporary housing until permanent housing is secured
  • Support clients with the lease-up process
  • Conduct comprehensive bio-psycho-social assessments
  • Develop individualized collaborative service plans
  • Assist clients with their physical and mental health needs by providing support and linkage to appropriate services.
  • Transport clients as needed to essential appointments
