Job Description
We have immediate temporary full and part-time assignments available with a full-service hospital in West Los Angeles, CA. The preferred schedule will be 8 am-5 pm M-F. The Social Worker will be rotating between inpatient patient adults, inpatient peds, outpatient nephrology, or any other departments within the hospital system.
Responsibilities include assisting patients and their families with care planning and discharge plans, completing psychosocial assessments, and partnering with patients to identify needs and develop and implement individual treatment plans based on a mutually agreed-upon treatment plan. In addition, SW will also discuss options for care proactively including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate.
Qualifications:
A master's degree in social work with an active ASW/BBS registration number with one (1) year of experience in a health care setting is a must. Knowledge of chronic and acute disease and how it impacts patient and family functioning is highly desired, Candidate must be computer-literate and, preferably, experienced in automated clinical information systems. *Our client requires all workers to have a completed Covid vaccine and booster.
The assignment pays $35.00 an hour.
