Job Description
PsychPros, Inc, has an immediate temporary, full-time assignment available for a master's level Social Workers with their ASW/ACSW with medical social work experience. Our client is a full-service hospital in West Los Angeles, CA.
The assignment pays $35.00 an hour.
Qualifications-
- A master's degree in social work with an active ASW/BBS registration number with one (1) year of experience in a health care setting is a must.
- Knowledge of chronic and acute disease and how it impacts patient and family functioning is highly desired,
- Candidate must be computer-literate and, preferably, experienced in automated clinical information systems.
- Our client requires all workers to have a completed Covid vaccine and booster.
Schedule-
- Monday-Friday 8am-5pm
Responsibilities-
- The Social Worker will be rotating between inpatient patient adults, inpatient peds, outpatient nephrology, or any other departments within the hospital system
- Assist patients and their families with care planning and discharge plans
- Complete psychosocial assessments
- Partner with patients to identify needs and develop and implement individual treatment plans based on a mutually agreed-upon treatment plan
- Discuss options for care proactively including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate.
