Job Description

We are assisting a growing behavioral health organization in its search for a full-time Pre-Release Case Manager in Richmond, VA.  Our client is dedicated to supporting individuals leaving prison on their healing journey, allowing participants to lead joyful, abundant, and purposeful lives. 

The Pre-Release Case Manager role serves as a trusted advisor for participants, supports the development of life goals, and builds relationships between participants and Concordance. The position plays a key role in supporting the successful re-entry of our participants by laying a solid foundation of skills and resources. As a Pre-Release case manager, you will facilitate individual sessions with participants within a correctional institution, identify their needs and goals, and connect them with the broader.

  • Skill in documenting interactions in case notes and flagging participant needs. 
  • Commitment to professionalism, integrity, and compassion in all interactions
 
Required education, licensure, and experience: 
  • Bachelor’s degree in social service-related field (social work, psychology, counseling, criminal justice, sociology, or similar)
  • 1-3 years of experience implementing case management services and documenting interactions
  • Driver’s license 
  • Ability to pass DOC security clearance. 
  • CPR / Adult First Aid certification preferred.
