Job Description
We are hiring for a FT RN or LPN in Everett, WA
The Supported Living Program (SL) provides community living services for adults with a wide range of disabilities and mental health challenges. The SL program promotes self-determination, independence, and competence for clients.
Duties & Responsibilities:
- Upon admission of the client, assess all health needs including mental health; formulate and implement a plan of care; and re-evaluate the care plan as needed.
- Coordinate all mental health care for all Residential/Community Living clients.
- Notify the Supported Living Program Director and Program Coordinator if significant changes in medical conditions occur which may potentially require additional support services.
- Provide instruction to clients and staff for the use of all prescribed medications and medical treatments including administering, storage, and side effects. On occasion will be required to pick-up, arrange for renewal, and disposal of outdated or no longer prescribed medications.
- Notify appropriate staff of changes in the client’ s medical needs and implement said changes
- Maintain charts and files for all residential clients on a constant and regular basis including, the documentation of all appointments, medications, correspondence, and conversations.
- Record observed or reported illnesses, accidents, or changes in functional abilities and dates of actions taken in the health record.
- Participate in any investigation of any accident or incident which jeopardized the health or life of a client and institute measures to prevent similar occurrences in the future as related to health matters.
- Notify the client’ s next of kin, guardian, or other individual or agency responsible for the client, as rapidly as possible, should a serious change in the client’ s condition, transfer of the client to the hospital, or death occur.
- Coordinate health care between the client, their chosen health professional, and their residential program.
- Advocate with health care professionals to acquire the best health care options for clients.
- Participate in specialized training and other professional development activities designed to update knowledge, enhance and increase skills, and foster professional growth.
- Interview, hire, and oversee the training of new Health Care Coordinators.
- Perform other duties as assigned.
Requirements:
- Education: Bachelors of Science in nursing or other relevant nursing education.
- Experience:

- Five years’ experience in human services or related fields required.
- Three years’ experience in providing health care and support to developmentally disabled, mentally ill, or physically disabled.
Shift:
Monday - Friday
8AM - 5PM
Compensation & Benefits:
- LPN: $35 - $45 Depending on Experience
- RN: $50 - $65 Depending on Experience
