PsychPros is currently partnering with a well respected community healthcare agency dedicated to safe and secure housing for clients in need of assistance. 

The rapid rehousing case manager will work with clients to obtain stable housing and ensure an ability to financially maintain housing. 

Job duties of the rapid rehousing case management position include:
  • Attend the Northern Kentucky Local Prioritization Committee meeting to receive referrals for the Rapid Re-Housing program, with a specific emphasis on medical considerations.
  • Collaborate with housing navigators to locate referrals, assess eligibility, and develop action plans addressing housing barriers and preferences.
  • Complete initial application, Vesta, and Treasury Portal intakes within 5 business days of participant entry, ensuring thorough documentation.
  • Conduct monthly in-home meetings to review budgets, bill payments, and case progress, ensuring habitable conditions.
  • Conduct in-home meetings specifically addressing participants' health-related needs and concerns, coordinating access to healthcare services, and empowering self-advocacy.
  • Provide transportation for participants attending medical appointments.
  • Complete quarterly and annual assessments within the Treasury Portal and Vesta database, ensuring timely documentation of inspections and necessary paperwork.
  • Maintain accurate and up-to-date documentation of case plans, encounter notes, and reporting forms.
Qualifications include:
  • Bachelor's degree in social service related field 
  • Minimum of 1 year experience working in social service field.
