Job Description

The School Based Mental Health Counselor / Social Worker is responsible for maintaining a client caseload in which they identify goals and objectives to help improve emotional, social and daily functioning. This position will be based on one of the following (3) school districts as a part of a System of Care Grant through SAMHSA; Saranac Lake Central School, Tupper Lake Central School, and Chateauguay Central School. While this position is located in the above schools, this employee is a Citizen Advocates employee and will maintain a 40 hour a week schedule and will provide services to the Malone or Saranac Lake outpatient clinic during school holidays, breaks, and summer vacation.

Essential Functions (Job Duties) 1. Complete a comprehensive biopsychosocial history assessment that includes the client’ s problems and strengths and preferences, including cultural, religious, and ethnic background. 2. Responsible for assisting clients in creating and maintaining person centered treatment plans every 90 days, that includes input from school staff, external stakeholders, and guardians. 3. Provide group therapy at the request of Practice Manager and/or school liaisons 4. Attends weekly staff meetings, treatment teams and supervision. 5. Completes mandatory documentation on EHR within timelines established by applicable regulations. 6. Empowers clients and encourages client’ s choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices based on racial, cultural and ethnic heritage, and sexual orientation. 7. Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, insurance eligibility, etc.). 8. Provides clients and families with crisis management services by appointment or walk-in. 2 /4 9. Develops a comprehensive discharge plan in collaboration with clients, clients’ families, multidisciplinary staff, and referral agencies. 10. Provide individual, group and/or family counseling for mental health and/or substance use. 11. Ability to collaborate with other local agencies (DSS, Probation, Drug Court). 12. Comply with Office of Mental Health (OMH), Office of Alcohol and Substance Abuse Services (OASAS), New York State and Federal regulations, laws and ethical guidelines
