ACSW Medical Social Worker-Temporary Assignment

Job Description
We have a full-time, temporary Medical Social Worker assignment available with a hospital in Moreno Valley, CA. The Social Worker will assist patients and their families with care planning and discharge planning, complete psychosocial assessments, and partner with patients to identify needs and develop and implement individualized treatment plans based on mutually agreed-upon goals. In addition, SW will also discuss options for care proactively, including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate. The SW may be assigned to all units in the hospital from ED, ICU, NICU, Labor & Delivery and Medi/Surg units.
The work schedule is M-F, 8:00 AM-4:30 PM. Prefer someone who is also available to work weekend shifts.
Qualifications:
Master's degree in social work accredited by the Council of Social Work Education with an active BBS registration as an ACSW with at least one (1) year post MSW experience in a health care setting. Hospital acute care experience required. Demonstrated ability to work on a multidisciplinary team. Must have solid psychosocial assessment skills. Knowledge of chronic and acute disease and how it impacts patient and family functioning. Must be computer-literate and, preferably, experienced in automated clinical information systems.
