Job Description
This position is an hourly, full-time, 40-hour-per-week position. A schedule can be Monday- Friday – 8 am to 4:30 pm or Sunday, Monday, Tuesday, Wednesday, Thursday - 11:30 am to 8:00 pm
Qualifications:
- Minimum BSW or a bachelor’s degree from an accredited institution in the social sciences field; or have a bachelor’s degree in any field and; at least two (2) years of experience providing case management services.
- Experience working with homeless individuals with chronic medical, mental health and substance abuse conditions.
- Housing Program Models and best practices like Housing First and focused model, Trauma-informed approach, Harm reduction strategies, motivational interviewing experience preferred.
- Continuum of Care and Coordinated Entry System experience, preferred.
- HMIS (Homeless Management Information System) experience, preferred.
- Dependable, responsible, and the ability to communicate effectively and respectfully.
- Ability to work independently and in a team environment.
- Excellent Time Management and Organization skills.
- Proficiency in Microsoft Word, Excel, Access and Outlook.
- Commitment to excellence, ethical behavior, and professional standards.
