Temporary ACSW Medical Social Worker

Job Description
PsychPros, Inc. has a full-time, temporary Medical Social Worker assignment available with hospital system in Los Angeles, CA, with travel required to skilled nursing facilities in Los Angeles County.
The Social Worker will assist patients and their families with care planning and discharge planning, complete psychosocial assessments, and partner with patients to identify needs and develop and implement individualized treatment plans based on mutually agreed-upon goals. In addition, SW will also discuss options for care proactively, including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate.
Qualifications: A master's degree in social work with an active BBS registration as an ASW with one (1) year of recent internship experience in a healthcare setting. Hospital acute care is highly preferred.
