Responsibilities-

Job Summary
Provides clinical assessment, psychotherapeutic interventions, and case management with patients/clients in the course of treatment. 

Job Specific Competencies
1. Conducts clinical intake assessments and develops treatment plans.
2. Provides clinical counseling and case management services to patients/clients:
a. Provides ongoing individual, family, or group counseling, using strengths and evidence-based approaches.
b. Documents clinical activity according to policies and in compliance with required timelines.
c. Provides case management services as needed.
(1) Links clients and supports to community resources.
(2) Coordinates treatment services and intervention strategies with internal and/or external services and supports, as appropriate.
3. As need and assigned will provide continuity of care through outreach services, including direct patient/client interventions at off-site locations such as local schools and community agencies. Job Specifications
1. Education: Master’s Degree in Social Work, Psychology, or Counseling.
2. Certification/Licensure: Licensed Mental Health Therapist (LMHC, LMFT, or LICSW) in WA State or eligible to obtain licensure upon hire. Valid Washington State Driver’s License with good driving record and access to a vehicle that is legally insured to be used for off-site clinical services consultation or case management. Basic Life Support required. All providers must be eligible for enrollment in Medicare and must not be currently opted out of the Medicare program.
3. Experience: Two years of mental health experience under the supervision of a qualified licensed professional to obtain licensure as indicated per RCW and WAC requirements.

The Addiction Counselor provides information, screening, assessment, individual and group counseling, and referrals to services with a patient-centered, trauma-informed, harm reduction approach with clients who have or may have a substance use disorder. This position works in collaboration with care coordinators, therapists, behavioral medicine consultants, psychiatric providers, and primary care providers in the provision of treatment services to support patients’ wellness and recovery. As need arises, the individual in this position may be required to provide limited days of service at the Chelan or East Wenatchee sites.

   Position Requirements:

• Education: Associate’s Degree in Chemical Dependency, Addiction studies, Human Services, or related field is required. Bachelor’s or Master’s Degree preferred.
• Credentialing: Meets Washington State RCW 18.205 and WAC 246-811 for certification as a Substance Use Disorder Professional and holds active WA Department of Health SUDP credential.
• Experience: Two years of providing substance use disorder treatment within a facility licensed to provide substance use treatment by the Washington State Department of Health, under supervision of SUDP/CDP Supervisor
• Language Skills: Bilingual in English/Spanish preferred

Job Summary
The Behavioral Medicine Consultant provides Primary Care Behavioral Medicine services including consultations to PCP’s regarding behavioral health diagnoses and recommended interventions to mitigate medical impact of behavioral health conditions. The Behavioral Medicine Consultant also provides direct service to patients that range from initial assessments, brief interventions, and less commonly intermittent long term therapy. Care is provided to a diverse population of individuals and families presenting to the clinic and is most often provided on the same day as the presentation. When assigned there may be opportunity to participate in the training of Psychology Interns and Postdoctoral Fellows.

Job Specific Competencies
1. Provides primary care Behavioral Medicine services to patients.
a. Coordinates Behavioral Medicine treatment services and intervention strategies with referring providers at and in some cased from the community.
b. Provides consultative visits and “warm hand-offs” with patients.
c. Provides assessment and brief therapy services, as well as rare intermittent and longer term services as indicated using evidenced based practices.
d. Utilizes individual and group sessions based on individual need.
e. Provides care in accordance to contract and treatment guidelines.
f. May provide ongoing care to actively hospitalized patients in hospital settings.
