The Case Manager will provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.<br />
<br />
This position has a 35-hour work week.&nbsp;<br />
Pay: $24.73 per hour.<br />
Schedule: MondayâFriday, 1:00 PM â 9:00 PM.<br />
Full-time with benefits.<br> 

<br>ESSENTIAL DUTIES AND RESPONSIBILITIES:<br />
--Provide direct services to a caseload of approximately 25 clients, including conducting necessary assessments, benefits assistance, and other services necessary to support the client in achieving housing and recovery goals<br />
--Conduct psychosocial evaluations.<br />
--Connect clients to psychiatric services.<br />
--Connect clients to medical treatment and substance use treatment facilities when applicable.<br />
--Obtain identification, public benefits, and other applicable income sources for clients.<br />
--Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans.<br />
--Prepare all documentation for housing applications.<br />
--Assist and counsel clients in the process of obtaining permanent housing.<br />
--Maintain record keeping in compliance with agency standards as well as those of City and State agencies.<br />
--Intervene in crisis situations.<br />
--Escort clients to appointments when needed.<br />

MINIMUM QUALIFICATIONS:<br />
--Bachelorâs Degree or equivalent work experience.<br />
--Two years of experience working with homeless adults and substance abuse issues preferred.<br />
--Experience with harm reduction and motivational interviewing techniques strongly preferred.<br />
--Valid NY, NJ, CT, or PA driverâs license with a good driving record.<br />
Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs.<br />
--Excellent writing and communication skills.<br />
--Bilingual Spanish/English is preferred.<br />

The Consumer Advocate works with people experiencing street homelessness and living with chronic medical conditions. The Consumer Advocate's objective is to connect individuals&nbsp;to appropriate social and medical services and, ultimately, to help improve their overall health and wellbeing and secure appropriate housing solutions.<br />

The Consumer Advocate&nbsp;builds a relationship with clients utilizing motivational interviewing to encourage clients to work towards self-identified housing, health, and other goals. The Consumer Advocate spends most of the shift in the field either in an automobile and/or on foot conducting street outreach, accompanying clients to appointments, and following up on service referrals. The Consumer Advocate conducts assessments, engages clients in self-directed service planning, and enters all required data into required databases.<br />

Pay: $20.70 per hour.<br />

ESSENTIAL DUTIES AND RESPONSIBILITIES:<br />
--Engage with identified persons currently experiencing homelessness and living with chronic medical conditions.<br />
--Draw on lived experience of homelessness, mental illness, substance use, and/or immigration issues to build rapport with clients and develop appropriate service connections.<br />
--Conduct required assessments.<br />
--Attend clinical supervision and training.<br />
--Maintain record keeping in compliance with agency standards as well as those of City and State agencies.<br />
--Collaborate with Parks Department, NYPD, DSNY, MTA, and other agencies.<br />
--Accompany clients to appointments and follow up on service referrals.<br />
<br />
MINIMUM QUALIFICATIONS:<br />
--High School diploma or equivalent required.<br />
--Minimum 2 years of related experience.<br />
--Lived experience with homelessness, mental illness, substance use, and/or immigration issues, and willingness to appropriately use lived experience as a tool for building client rapport and developing service connection preferred.<br />
--Knowledge of local benefit agencies (e.g., Public Assistance) and familiarity with substance use, mental health, immigration, and employment resources.<br />
--Strong oral and written communication skills and ability to liaise with internal and external stakeholders.<br />

As the practice continues to scale, expanding into new locations and growing its client base, it is seeking senior practitioners to support its next phase of development.<br />

<strong>The Role</strong><br />
The Senior Sports Psychologist / Performance Psychologist will deliver high-impact psychological services to elite performers, with a focus on optimizing performance, resilience, and mental readiness in high-pressure environments. This individual will operate as a trusted&nbsp;performance advisor, working closely with clients to develop tailored strategies that enhance consistency, focus, and competitive&nbsp;outcomes.<br />

<strong>Key Responsibilities</strong>
<ul>
&#9;<li>Provide individualized performance psychology interventions, drawing on evidence-based modalities such as CBT, ACT, and mental skills training.&nbsp;</li>
&#9;<li>Work with professional and elite athletes, as well as high-performing individuals across domains.</li>
&#9;<li>Design and implement mental conditioning programs focused on performance optimization, recovery, and stress regulation.</li>
&#9;<li>Collaborate with coaches, trainers, and multidisciplinary professionals to support holistic performance outcomes.</li>
&#9;<li>Deliver workshops, presentations, and educational programming.</li>
&#9;<li>Support clients navigating injury, transition, performance anxiety, and high-stakes competition.</li>
&#9;<li>Uphold the highest standards of confidentiality and professional integrity.</li>
</ul>
<strong>Candidate Profile - Experience</strong>

<ul>
&#9;<li>10+ years of experience in sport and/or performance psychology.</li>
&#9;<li>Demonstrated success working with elite or professional athletes.</li>
&#9;<li>Strong grounding in both clinical practice and applied performance psychology.</li>
&#9;<li>Experience in team, organizational, or other high-performance environments is preferred.</li>
</ul>
<strong>Candidate Profile - Credentials</strong><br />
Ph.D. or Psy.D. in Clinical Psychology, Sport Psychology, or a related discipline.<br />
Licensed Psychologist in the State of New York.<br />
